Job Summary
A care coordinator ensures client are placed with the right caregiver (aide) and receive services in a timely manner. Handling any issues and maintain continued client satisfaction.
Duties includes:
Handling all client staffing issues effectively and timely
Handle all Scheduling/Rescheduling of clients and can(s).
Handling all call outs promptly and effectively
Onsite visitation as needed for paperwork and conflict resolutions
Communicate and report daily as applicable any needed follow ups with case managers, MDs and social workers.
Handle and resolve all client and staff complaints/concerns.
Carry out performance evaluation on all staffs (CNAs, LPNs, RNs) twice a year
Be on call during work hours when needed.
Manage EVVs
Hire staffs for cases
Qualifications:
Prior caregiving experience is a bonus
Candidate must have excellent interpersonal and communication skills.
Good computer skills is needed.
Ability to multitask and self-start.
Training will be provided to the qualified candidate.
